How to Build a Survey

Design employee surveys in the visual builder, pulse checks, onboarding touchpoints, and exit interviews, then distribute them through deployments or lifecycle automations.

Three new engineers started Monday. Priya needs a week-1 onboarding pulse live by Thursday so managers can act on feedback before day ten, not after someone quietly disengages.

Before you begin

  • DEX license enabled for WorkVerge
  • At least one employee folder if you plan to deploy internally

Overview

Surveys are the foundation of DEX. You build them once in the survey designer, then reuse them across deployments, onboarding checkpoints, and exit interviews. Pull logos and header images from the Media Library so every touchpoint feels on-brand, not a generic form vendor.

Priya's week-1 pulse

Survey name: Week 1. How's it going?
Questions: 1–5 onboarding clarity rating, NPS-style "Would you recommend Bluewave as a place to work?", open text "What's one thing we could fix this week?"

Build your survey

    Open DEX → Surveys

    You'll see every survey your org has built, drafts, published, and archived.

    Click Create Survey

    Give it a clear name and short description. Add company branding from the Media Library if you want a consistent look.

    Add blocks and questions

    Mix NPS, rating scales, multiple choice, and open text. Mark critical questions as required so you don't get half-empty submissions.

    Configure branching

    Show follow-up questions only when someone picks a low score, e.g. "What would improve your first week?" after a 1–3 rating.

    Preview and publish

    Walk through the survey as an employee would. When it reads well, publish and attach it to a deployment or lifecycle flow.

Question types: multiple choice, multi-select, rating scales, NPS, short text, and long text. Use NPS for quarterly engagement; use branching on low scores for onboarding and exit flows.

How to send it

  • Deployments. Best for targeted sends. Each folder member gets a unique link; you track opens and completion.
  • Onboarding. Fires automatically when HR marks someone as a new hire.
  • Public share link. Useful for anonymous feedback; authenticated employees can also open surveys from the Employee Portal.

When do reminders go out?

Reminders are configured on the deployment, not the survey itself. Set start and end dates plus reminder cadence when you launch, see Deployments.

Lifecycle surveys

The same survey designer powers three programs Priya runs every quarter:

  • Week-1 onboarding pulse. Short, actionable, sent via Onboarding
  • Quarterly engagement survey. Longer NPS + department themes, sent via Deployments
  • Exit interview. Confidential open text, triggered from Offboarding
